Problem with a purchase shown on your statement: Credit card

I am submitting this complaint because I am the victim of identity theft and fraudulent activity that resulted in unauthorized inquiries and/or accounts appearing on my consumer reports. Pursuant to the Fair Credit Reporting Act ( FCRA ), including 15 U.S.C. 1681e ( b ), 1681i, and 1681c-2, consumer reporting agencies and furnishers are required to maintain reasonable procedures to ensure maximum possible accuracy and to block information resulting from identity theft. I notified the reporting agencies and provided supporting documentation, including my identity theft report and proof of identity. Despite this, inaccurate information continues to be reported, causing harm to my creditworthiness, reputation, and ability to obtain credit. I request the immediate removal and blocking of all fraudulent information related to identity theft, a complete reinvestigation of the disputed items, and written confirmation that the inaccurate information has been permanently deleted from my consumer reports in compliance with federal law.
