I am reporting identity theft because accounts and/or debts were reported under my name that I did not authorize or create. I discovered these unauthorized accounts when reviewing my financial records and credit information. I dispute responsibility for these accounts because they were opened or used without my knowledge or permission.

Im writing to file a complaint against IC SYSTEM INC for violating my consumer rights under 12 CFR 1006.34 and 15 USC 1681-s2 ( 7 ) ( A ). Theyve added an inaccurate account to my consumer report without giving me the required validation info, as outlined in 12 CFR 1006.34 ( b ) ( 5 ). On top of that, I havent been given a chance to dispute the account, which Im entitled to under 12 CFR 1006.34 ( c ) ( 4 ) ( i ). Overall, their actions are not only against federal regulations but also misleading and deceptive.

This debt does not belong to me. It belongs to a deceased family member who had the same name. I request that you investigate this matter and remove this account from my credit file unless you can provide documentation proving that I am legally responsible for this debt.

I'm writing to formally request that the collection account mentioned in your most recent email be validated. In accordance with my rights under 15 U.S.C. 1692g ( b ) of the Fair obligation Collection Practices Act ( FDCPA ), I ask that your office furnish comprehensive and verifiable proof attesting to the validity of this obligation. At the very least, this validation has to contain : The original creditor 's complete name and address, A thorough statement of the whole amount owing, including principle, interest, fees, and any additional charges, the original account number, a full payment history and evidence, such as a signed contract or other official papers, proving I am legally required to pay this obligation. You are legally obligated to stop all collection efforts and not report this account to any credit reporting agencies until such validation is received. This inquiry is meant to make sure the data you are reporting is correct, legal, and not the product of fraud, miscommunication, or error.

To whom it may concern, Please note that this is not a refusal to pay off the account but a notice sent as per the Fair Debt Collection Practices Act ( F.D.C.P.A. ), 15 U.S.C. S 1692 ( G ) Section 809 ( B ). This notice states that your claim is disputed and validation of the account is required. This notice is not a request for verification of the account or proof of my mailing address but a request for validation made pursuant to the F D.C.P A. laws. I humbly request that your agency/ office sends me valid proof of claim that I am legally obligated to pay you. Please provide me with the below mentioned items : * The amount you claim the respondent owes you * An explanation showing me how you have computed the amount. * Copies of any documents that prove I agreed to pay the alleged amount * The identity of the original creditor * Confirmation that the account has not crossed the statute of limitation ( SOL ) period * Evidence that you are a licensed debt collector * Evidence of your license numbers and your registered agent. * Chain of Title of ownership of the debt * Error & Omissions Insurance

I am writing to formally request debt validation for a collection account mentioned in a letter I recently received. Under my rights through the Fair Debt Collection Practices Act ( FDCPA ), I ask that your office provide clear proof that the debt is valid. This should include the name of the original creditor, the amount owed, a full payment record, and proof that I am responsible for this debt. I am making this request to ensure the information being reported is accurate and not the result of a mistake, fraud, or confusion. While waiting for proper validation, I ask that you pause all collection activity and reporting related to this account, as allowed by law.

I never consented to the release of my medical information to this debt collector. Because the medical account contains private information, it should not be reported on this account because you are infringing on my privacy. This is a clear HIPAA violation due to its failure to comply with any aspect of HIPAA standards and provisions detailed in 45 CFR Parts 160, 162, and 164. I demand this account be deleted from my credit report immediately.

I am submitting this complaint because a debt collector is attempting to collect and report an alleged debt without providing lawful validation as required under the FDCPA ( 15 U.S.C. 1692g ), Regulation F ( 12 CFR 1006.34 ), the FCRA, and New Jersey state laws, including the NJ Consumer Fraud Act and the NJ Consumer Finance Licensing Act. I formally disputed the alleged debt and requested full validation, including : identification of the original and current creditor, a signed contract or compliant electronic agreement, itemized accounting of all amounts, complete chain of assignment/bills of sale listing my specific account, original statements, proof of charge-off, proof the debt is within the New Jersey statute of limitations , proof of an active NJ debt collection license, and evidence their initial notice met all Regulation F disclosure requirements. The collector has * * not provided legally sufficient verification * *, and instead continues credit reporting and/or collection activity. Reporting and collecting on an unverified debt violates FDCPA 809 ( b ) and FCRA 623 ( a ) ( 3 ), as well as New Jersey laws prohibiting deceptive or unconscionable practices under N.J.S.A. 56:8-2. Until full validation is provideddirectly from the original creditor, not internal summariesthe collector is required to cease all collection efforts and stop furnishing negative credit information. Failure to do so constitutes continued violations of federal and state law. I am requesting CFPB assistance to : 1. Require the collector to provide full and complete validation ; 2. Ensure they cease collection until validation is supplied ; 3. Correct or remove any unverified credit reporting ; and 4. Ensure compliance with FDCPA, Regulation F, FCRA, and New Jersey statutes.

This debt collector has engaged in conduct that violates the Fair Debt Collection Practices Act ( FDCPA ). They are reporting an account that I did not authorize, and no signed agreement exists bearing my signature to validate any obligation. A legally binding contract requires clear consent from all parties, which is absent in this case. Additionally, the collector has failed to provide proper validation of the alleged debt as required under the FDCPA. Without sufficient documentation verifying the debts legitimacy, their continued reporting and collection efforts are unlawful. These actions constitute violations of my consumer rights. As a result, I demand the immediate deletion of this account from all reporting agencies. Failure to comply may result in further legal action, including claims for damages.
