I am a victim of identity theft. I am writing to request that you delete the following fraudulent inaccurate information from my credit report. Someone has used my personal information to obtain loans and credit cards. I have no idea how the theft took place. I also have no knowledge of any suspects. I did not receive any money, goods, or services as a result of identity theft. Please let me know if you need any other information from me to block this information from my credit report.

There are collection accounts on my report that I believe contain inaccurate information. Under my rights pursuant to 15 USC 1681e ( b ) and 15 USC 1681i, I am entitled to an accurate credit report. I request a review of these entries, and if they can not be verified as accurate, I ask that they be removed.

Violation of 15 U.S.C. 1681c ( a ) ( 5 ) : No adverse items of information should appear on my report if they date back more than seven years, unless they are criminal convictions. These items are outdated and must be removed immediately. I am writing to formally dispute false, inaccurate, and unverifiable information appearing on my consumer credit report. After multiple reviews and failed attempts at proper reinvestigation, it has become clear that your continued possession and transmission of this data constitutes criminal misuse of my personal identity under 18 U.S. Code 1028A Aggravated Identity Theft. Your agency is using my name, Social Security number, date of birth, and other identifying information to distribute false and unverifiable data, such as alleged late payments, incorrect balances, and other negative account information. Despite prior notices disputing these entries, you have continued to furnish this data without producing : Any original contracts or applications, Complete payment records, Or legally admissible proof of the validity of the derogatory entries. As a result, you are knowingly publishing and transmitting false data tied to my personal identity without lawful authority. The continued use of unverifiable information attached to my identity after dispute, and your failure to lawfully validate it constitutes aggravated identity theft under federal law. While some accounts may have originated from me, the inaccurate negative information being published now is false, lacks verification, and is being attached to my identity in a misleading and harmful manner. You are knowingly attributing inaccurate financial behavior to me, damaging my reputation, and using my PII to falsely profile me in violation of federal and consumer protection statutes.

I want to make sure that everything is accurate, therefore I want you to look into it for me. I am the primary consumer of the complaint and there is no third party involved. This account is currently reporting late payments that are inaccurate and do not reflect my true payment history. The reported delinquencies are causing ongoing damage to my credit profile. Under the Fair Credit Reporting Act ( FCRA ) 607 ( b ) and 1681i, credit reporting agencies and furnishers are required to maintain maximum possible accuracy and conduct a reasonable investigation when information is disputed. I am a victim of identity theft. Attached are my FTC Identity Theft Report, identity documents, and legal demand under FCRA 605B ( 15 U.S.C. 1681c-2 ). I demand the immediate deletion of all fraudulent accounts listed in my letter within 4 business days, as required by federal law. Failure to comply will result in me filing formal complaints with the CFPB, FTC, State Attorney General, and pursuing civil litigation for willful noncompliance under FCRA 616 & 617. You are legally obligated to block and permanently delete this information. Any refusal, failure, or delay will be treated as intentional violation of my consumer rights, exposing Equifax to statutory, actual, and punitive damages, including attorneys fees. This complaint arises from the consumer reporting agencys failure to provide a complete and legally sufficient Method of Verification ( MOV ) in response to my dispute and request for reinvestigation. Pursuant to the Fair Credit Reporting Act ( FCRA ), including 15 U.S.C. 1681i ( a ) ( 6 ) and 1681i ( a ) ( 7 ), I requested a detailed explanation of the procedures used to verify the disputed information appearing on my consumer report, including the specific method of verification employed, the identity of any furnishers or individuals contacted during the investigation, and the documentary evidence relied upon to substantiate the accuracy and completeness of the reported information. Despite this request, the consumer reporting agency has failed to provide the requested information or any competent and verifiable evidence demonstrating that it conducted a reasonable and legally compliant reinvestigation. The FCRA imposes an affirmative duty on consumer reporting agencies to conduct a reasonable reinvestigation of disputed information and, upon request, to disclose the method of verification used to confirm the accuracy of the information being reported. A conclusory statement that an account was simply verified does not satisfy these statutory obligations without supporting evidence explaining how the verification was performed, what procedures were followed, and what records or documentation were reviewed. In the absence of competent and verifiable documentary evidence establishing the basis for the agencys verification of each disputed item, the continued reporting of such information undermines the accuracy, reliability, and integrity of my consumer credit file. Moreover, the agencys continued reporting of disputed information while failing to provide the requested verification details or supporting documentation demonstrates an apparent failure to maintain reasonable procedures to assure the maximum possible accuracy of consumer information, as required by 15 U.S.C. 1681e ( b ). Information that can not be substantiated through competent and verifiable records should not continue to appear on a consumer report. The agencys failure to provide a legally sufficient Method of Verification and supporting evidence is inconsistent with its obligations under the FCRA and warrants regulatory review and appropriate corrective action, including the deletion or correction of any information that can not be properly verified through competent documentary evidence.

I need you to certify, with full accuracy and confidence, that every single piece of data tied to this account actually belongs to me including but not limited to the account number, payment history, balances, dates, and all identifying details. If you can not fully verify and certify that this account and all of the information attached to it is 100 % accurate and legitimately tied to me, then you are obligated to delete the entire account from my credit report immediately. Let me be very clear : Im not asking whether the account looks familiar. I am demanding that you confirm that each data point reported under my name is accurate, verifiable, and truly mine. If theres even a shred of uncertainty, or if you can not produce the necessary records to validate this account as mine, it does not belong on my credit report and keeping it there would be a violation of my rights under the Fair Credit Reporting Act ( FCRA ).
